Rectus Abdominis

Patient position:
supine on a firm surface with forearms folded
across the chest. No stabilization should be
applied to the trunk. Assist the patient in
bringing the legs to a vertical position (90
degrees hip flexion with knees fully
extended). Flatten the low back against the
table by tilting the pelvis posteriorly. Maintain
this pelvic position while lowering the legs.
Do not allow the patient to raise the head and
shoulders. Observation of the pelvis and low
back positions determine the grade of the
test. (Note: photograph does not show anti-
gravity test position.)
Examiner pressure:
The force exerted by the hip flexors and the
lowering of the legs tends to tilt the pelvis
anteriorly to act as a strong resistance to the
abdominal muscles. As the legs are lowered,
the leverage increases.
Strength is graded based on the ability to
keep the low back flat on the table while
lowering both legs from vertical (90 degree
angle). Note the angle between vertical and
the extended legs at the moment the pelvis
begins to tilt anteriorly.
Normal: 0-15 degrees (horizontal to 15
degrees)
Good: 15 degrees to 45 degrees
Fair: 45 degrees to 90 degrees (vertical)
