THE EFFECTIVENESS OF MOLSIDOMIN IN PATIENTS WITH ACUTE MIOCARDIAL INFARCTION

The aim of this study was the evaluation of influence of molsidomin-retard on the indexes of central hemodynamics, tolerability of stress and clinical symptoms in patients with acute Q-wave MI. These patients did not get the thrombolytic therapy.
We have studied 59 patients (mean age 57,4+/-4,1 years), which were taken in the ICU after 6-12 hours from the beginning of Q-wave acute MI. After 48 hours of continuos nitroglycerin infusion the patients were divided in 2 groups: patients from the 1-st group (37) were taking molsidomin 8 mg every 12 hour; patients of the 2-nd group (22) were taking 40 mg of JSDN every 12 hours. On the 3-5th day of treatment we performed EchoCG, on the 21-28th day - EchoCG and stress-test. In both groups we have found the same positive influence of molsidomin and JSDN on EF (p<0,05 comparing with 3-5th day of treatment) and on LVEDV (p<0,05 in the 1-st group. p<0,05 in the 2-nd group).
When the patients became more active on the 21th day after MI, in 8,1% of patients from the 1-st group and 31,8% of patients from the 2-nd group appeared anginal attacks more confirmed by ST depression on Holter-monitoring. We think, that this was the result of tolerance development to nitrates in the 2-nd group and correlated with data of stress-test in these patients. When the patients from the 2-nd group were given the combined antianginal therapy (JSDN 60 mg in the morning and molsidomin 8 mg in the evening) we got effective results such as increasing of tolerance to stress, decreasing of anginal attacks and the frequency of reduction-syndrom.
Molsidomin is effective antianginal medication for patients with acute MI and can be alternative and adequate adjunctive to nitrate therapy.
