Rehabilitacja więzadła krzyżowego
The rehabilitation protocol is flexible, intensity and progression of rehabilitation vary and is rełated to: the surgery (graft choice, fixation, operative procedurę), timing from injury (tissue adaptation, com-pensatory mechanisms), coexisting injures (menis-cus and cartilage lesion), age, patients activity level, motivation and knowledge of the rehabilitation program.
Principles of postoperative care and rehabilitation in our clinic are:
— decrease of pain, swelling, and inflammadon
— fiill weight bearing as tolerated witch or without cratches
— immediate ROM exercises in the rangę 0° - 60° to 90° as tolerated, gradually inereased to 120° and fuli flexion after 6-9 weeks postoperatively (utilizing CPM, passive, active assisted and active exercises)
— quadriceps, hamstrings and whole lower limb and pelvis muscle exercises with inereasing resistance in close and open kinetic chains
— prevention from patello-femoral joint problems
— proprioception and coordination exercises (from early postoperative)
Uszkodzenie więzadła krzyżowego przedniego (WKP), jako jedno z najcięższych uszkodzeń aparatu więzadłowego stawu kolanowego, w większości przypadków wymaga leczenia operacyjnego i specjalistycznej rehabilitacji.
Leczenie zachowawcze wybranych pacjentów (spełniających odpowiednie kryteria) daje zadowalające rezultaty, ale zmuszeni są oni do obniżenia poziomu, a nawet rezygnacji ze swojej aktywności sportowej, i do ciągłej kontroli funkcjonowania stawu kolanowego (15, 27).
Rehabilitacja po rekonstrukq‘i WKP budzi wciąż wiele kontrowersji. Dotyczą one najczęściej: stopnia ograniczenia zakresu ruchu oraz czasu rozpoczęcia obciążania kończyny we wczesnej fazie pooperacyjnej, metodyki i intensywności ćwiczeń usprawniających, jak również czasu powrotu pacjentów do normalnej codziennej aktywności, a później aktywności rekreacyjnej i sportowej. Niemal wszystkie ośrodki, w których wykonywane są rekonstrukcje więzadeł, stworzyły własne procedury postępowania pooperacyjnego i usprawniania,

We focus on achieving as fast as possible, fuli exten-sion of the knee, muscle control and proprioception. Exercises are perfbrmed in closed and open kinetic chains, with cocontraction of hamstring muscle, in the way to avoid too much stress on heałing graft.
In cases of athletes and reereationał active patients, there were included to rehabilitation program (from early postoperative) elements of sports specific exer-cises.
That helped to restore proper movements patterns and to brake psychical barrier in retuming to fuli sports activity.
Return to sport is allowed when patients achieves fuli rangę of motion, normal proprioception and ba-lance, functional tests results about 90% and there is no pain or swelling in connection with physical ac-tivity. [Acta Clinica 2:86-100]

które czasem znacznie się różnią, lecz opierają się na tych samych podstawowych zasadach, mających na celu ochronę przeszczepu i powrót pełnej funkcji stawu kolanowego bez utraty jego stabilności (5, 10, 17, 18, 22, 23). Wyniki najlepiej przeprowadzonej operacji mogą być zniweczone przez późniejsze niewłaściwe postępowanie pacjenta lub nieodpowiednią rehabilitację.
Głównym celem rehabilitacji po rekonstrukcji WKP jest przywrócenie pełnej funkcji stawu kolanowego i całej kończyny dolnej (tj. odzyskanie prawidłowej ruchomości, propriocepcji, siły i koordynacji) oraz możliwie jak najszybszy powrót pacjenta do aktywności dnia codziennego, pracy oraz aktywności rekreacyjnej czy sportowej, bez utraty stabilności stawu. Zadaniem rehabilitacji jest również zapobieganie powtórnym urazom i przeciążeniom innych części ciała, które kompensują niewydolność kolana w łańcuchu kinematycznym podczas różnorodnej aktywności
